Scalability may not be a huge issue if we apply sharding techniques. As for security, I think zero-knowledge proofs can play a crucial role in allowing secure authentication and giving proper credentials to the parties involved in patient care.

The key issue I see lies within data interoperability. There are so many different systems, protocols, and formats in healthcare, it'd be hard to integrate all of them with a decentralized system.
